Well, these aren't exactly polka dots. I actually use a piece of the Golden Honey Specialty Designer Series Paper that is free during Sale-A-Bration with a qualifying order.
A very simply Z fold card, I used a butterfly for retired DSP but anything could be used to decorate the front of the card.
